Before submitting a change of address, please enter your postcode into the search box on our Practice Boundary page. The search will indicate whether your new address remains within our catchment area.  We will NOT be able to update your record if your new address is outside our boundary. 

It is important that your medical record contains your correct address. A discrepancy of address may mean that you do not receive important information including hospital appointment notifications/clinic letters.
